HIPAA Privacy Policy

THIS POLICY DESCRIBES HOW YOUR PROTECTED HEALTH INFORMATION (”PHI”) MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION.

Sentrian (referred to as “Sentrian” or “we,” “our,” or “us”) is a healthcare company that leverages advances in biosensor technology and machine intelligence in order to make remote patient intelligence an affordable and effective reality for patients, payers, and providers. This Sentrian Privacy Policy (this “Policy”) describes the rules that govern our collection, storage and use of Personal Information (defined below) gathered while we provide our Remote Patient Intelligence platform (Sentrian RPI) Services.

Introduction

This Policy addresses our use of Personal Information in connection with our provision of Sentrian RPI (We refer to these collectively as the “Services”).
We may receive Personal Information in connection with our provision of Services, and this Policy describes how we may use that information.
“Personal Information” means information that identifies or that could be used to identify a patient or Trial Subject, such as contact information (e.g., name, address, email address, or telephone number), account number, and any other non-public information that is associated with such information and patient or Trial Subject. Personal Information may include information regarding the person’s health.

How We Collect Your Information

Sentrian and its employees and agents collect PHI through the Sentrian RPI. We are required by law to maintain the privacy of your PHI and to provide you with notice of our legal duties and privacy practices with respect to PHI. We are required by law to abide by the terms of this policy and to notify you of any material changes in our policy.

What We Do Not Do With Your Information

Information about your financial situation and medical conditions and care that you provide to us (directly or indirectly given to us) is held in strictest confidence and used only in connection with remote patient monitoring. Once we have your information in our system, we encrypt it and employ other security measures to ensure it remains confidential.
We do not give out, exchange, barter, rent, sell, lend, or disseminate any information about patients or clients that receive our services that is considered patient confidential.

How We Do Use Your Information

Information is only used as is reasonably necessary to process Sentrian RPI in an analytics platform.

Information We Do Not Collect

We do not use cookies on our website to collect data from our site visitors. We do not collect information about site visitors. We do use some affiliate programs that may or may not capture traffic data through our site. To avoid potential data capture, simply do not click on any of our outside affiliate links.

Limited Right to Use Non-Identifying Personal Information

Any pictures, stories, letters, biographies, correspondence, or thank you notes sent to us become the exclusive property of Sentrian.
You may specifically request that NO information be used whatsoever for any purpose, but you must identify any requested restrictions in writing. We respect your right to privacy and assure you no identifying information that you send to us will ever be publicly used without your direct or indirect consent.

Your rights with respect to your PHI

You have the right to a) request restrictions on certain uses and disclosures of your PHI; b) receive confidential communications regarding your PHI; c) inspect and copy any of your PHI which we hold; d) receive an accounting of any disclosures we may make of your PHI; and e) obtain a paper copy of this policy from us upon request.

Complaints
