First, in collaboration with our clients we identify patients at risk for hospital admission and whose course can be influenced.  Then, Sentrian provides the appropriate biosensors and wearable devices, emphasizing lower cost and less intrusive devices, based on the risk assessment for each patient.  The patients receive instruction and support on the use of the devices to simplify engagement.  Our platform makes it easy for the clinicians to create rules that, based on deviations in the measured parameters, prompt automated notifications to the care managers.  The rules address both short term and longer term changes, since health deterioration patterns frequently develop as much as ten days before a patient becomes symptomatic.

Sentrian’s big data predictive analytics platform helps personalize the notification process, is intended to reduce false alarms and, through feedback and machine learning improves with time.  It’s intended to allow qualified care managers, a scarce resource, to help more patients by reducing the wasted labor associated with false alarms.  More effective interaction between the patient and the care team improves outcomes, reduces avoidable costs and improves patient satisfaction.  Satisfied patients means higher HEDIS and Star ratings and makes it more likely that the patient will stay enrolled with the health plan or provider.

These notifications are designed to provide care managers and clinicians with enough time to intervene with safer, lower cost, treatment options that keep patients healthy and happy at home.
